What is Outcomes-Based Support Planning?

Outcomes-Based Support Planning focuses on the results you want from your care, not just the services you receive. It involves working with you to understand your goals and aspirations—both big and small—and then tailoring support to help you reach them.

These goals might include:

  • Regaining confidence after illness or injury
  • Maintaining independence at home
  • Being able to cook your favourite meal again
  • Building friendships or reconnecting with family
  • Getting out and about more in your community

What This Means For You

You are at the centre of everything. With our approach:

  • Your voice matters: We listen closely to understand what matters most to you.
  • Your care plan is personal: Together, we co-create a support plan based on your own goals and preferences.
  • Progress is reviewed regularly: We work with you to track your progress and make changes when needed.

How We Work With You

  1. Initial Conversation
    We meet with you (and anyone you want to be involved, like family or advocates) to talk about what a good life looks like for you.
  2. Setting Outcomes
    We help you identify outcomes that are specific, meaningful, and achievable. We focus on what you can do and want to do, not just what support you need.
  3. Planning Support
    Our team works with you to design support that fits around your life, routines, culture, and choices.
  4. Delivering Flexible Support
    Our carers are trained to deliver flexible, respectful support that adapts as your needs and goals evolve.
  5. Monitoring & Review
    We agree on how often to check in and review your progress together. We celebrate success and make changes where needed.

Your Rights & Involvement

  • You have the right to be fully involved in planning your care.
  • You can involve others if you wish.
  • You can request changes to your plan at any time.
  • We will communicate clearly and respectfully.

Examples of Outcomes-Based Support In Practice

  • George, 76, wanted to rebuild confidence walking to the shop after a fall. His care plan included daily encouragement and physiotherapy exercises. After 8 weeks, he achieved his goal.
  • Nazia, 43, was feeling lonely and wanted to reconnect with her sister. Her carer supported her to set up regular video calls and helped her attend a local women’s group.
  • Irene, 89, was finding it hard to cook meals. Her support now includes joint meal preparation sessions, helping her enjoy favourite recipes again.
